Not always the same but mostly..
ME1: Garrus & Wrex - Good mix of Tech, Biotic and Combat skills, very versatile and badass.
ME2: Garrus & Miranda - Miranda's extremely balanced and comes with the nice passive.
Garrus is a powerhouse once he gets Tungsten rounds and some Sniper upgrades.. And with his passive his AP ammo, sniper and Overload hurts even more, add in Miranda's passive and you're good to go.
Plus they're my two favorites of the ME2 squaddies.

ME1: Ashley, Garrus, Tali. Mostly it was Ashley and Tali.
ME2: Used to be Zaeed and whoever I felt I needed at the time. 90% of the time it was Miranda until Kasumi came around. Now it's Zaeed and Kasumi all the time. Kasumi's cloak attack gives me some method of crowd control. It's also good with keeping her alive if her health is low. Have her go after an enemy and on the way back I direct her to cover. Her grenade is great too.
I've learned to not tell Zaeed where to go for cover. He knows best what to do...most of the time. Only time I tell him to take cover is before the battle takes place and if he's still standing in front of a cover spot and just standing there taking damage. Also add, disrupter ammo, concussion shot, and inferno grenade and that's it.

ME1
If I have tech skills then it's Ashley and Wrex.
If I am a Biotic then, Ashley and Tali.
If I am a Soldier then Liara/Wrex and Tali. Liara will swich out with Wrex depending on my mood.
ME2
Miranda has become a staple of my crews, ie she is always in my squad except for a very very very few where I try out different combos ie Grunt and Jacob (dual inferno and plenty of armor bonus ) for husk mine mission.
The other spot is as follows:
Shield heavy missions get Kasumi as the second.
Husk heavy missions get Grunt.
Collector platform missions get Thane.
Legions mission is also a special case, leading to Legion and Garrus for dual snipers because of the way I play the wave attacks during the Viral download.

Grunt+X for me pretty much, mostly because i'm lazy and I find he's probably the most effective squadmate with abilities on auto. Hes ridiculously durable and his hp upgrades just make him even more so, so I hardly need to control him unless he gets himself stuck on something, and he's great cover for any enemies who get close. 3rd slot is based off whatever my class is or if I feel like i'm neglecting a certain squadmate

No matter what I play, I always have Miranda in my party. Her group bonuses, plus utility is top notch.
15% weapon damage is great for soldiers/infiltrators, and 15% hp is nice for stronger difficulties, if you need it.
The other member depends on what I will be fighting for that mission.
Mechs/geth - Zaaed, Garrus, Kasumi.
Blood pack - Mordin, Grunt.
Eclipse - Same for mechs/geth.
Blue Suns - Same for mechs/geth.
Collectors - Mordin, Grunt, Samara.

Legion and Garrus/Thane (for long range missions)
Miranda/Thane and Mordin (taking out vorcha and krogan)
Grunt/Jacob (taking out husks, provided barrier and fortification are on)
Garrus/Zaeed and Miranda (for blue shielded enemies and geth/mechs)
Samara and Miranda/Thane (for warpsplosions)
I get all passives maxed out, give all snipers incisors and get all ammo and drone abilities activated.

My favourite two to bring with me in Mass Effect 1 for personalities sake were Ashley and Wrex. Both were very tough and both exchanged quips and as a result were a joy to bring along. Honestly from where I stand they were practically kindred spirits, despite that whole Virmire episode.
Plus Wrex was the only male on my team (in the end) who had absolute faith in Shepard and likewise Ashley seemed very loyal to Shepard, backing him up in arguments and of course she and he were planning to sleep together.
In Mass Effect 2 my favourite pairup was Tali and Garrus. Partly because they were familiar faces from Mass Effect 1 and Tali was like Shepard's little sister and Garrus was like his best friend. But mostly it was because they too had great personalities that bounced off each other really well.
Honestly if all these guys and gals were real I'd probably want to actually hang out with them.
EDIT: Also for the most part I only cared about personality not tactical use so that featured a lot in my choice of who to bring with me, that said I was obviously tactical in the Suicide Mission and luckily no one was lost.

ME1 = Garrus and Wrex (sometimes ashley)
ME2 = Garrus and Thane (sometimes Grunt/Mordin/Legion)
Garrus and Thane is the absolute best combination for an infiltrator.... for a soldier, Grunt and Thane is also a very good combination. But in ships, when there's like a corridor and in the end you got all the enemies, it's good to have Legion as a sniper and yourself and garrus with assault rifles.

ME1 = Wrex + Liara, sometimes Garrus, but not very often
Wrex - simply the best character in ME, imho of course
Liara - like her a lot, has singularity
Ashley, Tali and Kaidan stay on the ship all the time
ME2 = Miranda + Grunt/Samara/Thane/Mordin/Legion/Garrus/Kasumi/Zaeed
I take Miranda all the time - looks great, provides best bonuses
I pick the second squadmate randomly, though I tend to use Legion and Grunt more often than other characters.
I dislike Tali, Jack, Jacob - I never take them on missions unless absolutely necessary (loyalty).

ME1: Garrus and Tali every time.
ME2: Depends who I'm fighting.
Normally though either Garrus, Miranda or Mordin. Occassionally Thane or Legion. It depends what skills I need.

Yes! I loved that combo my first playthrough. Its like having 5 character fighting!Lemonwizard wrote...

Legion/Tali for life!
ME1:
always Kaidan. I love his voice, so anything he had to say was always welcome. He has overload, which is useful for my Adept. And I like fighting from a distance with powers more than with the guns. He also could help fix the Mako.
I'd mix up the second person depending on what the situation was. But Ashley played well and had some fun dialogue with Kaidan. So I liked having her along.
ME2:
First time through I usually had Garrus and or Tali with me often because I didn't trust Cerberus and knew I could trust them. I know, it doesn't quite work that way in the game but hey! Its role play.
I didn't like Miranda the first time through, but this time am finding what a gem of a squadmate she is with her overload and her biotics.
If I don't take Miranda, I often take Kasumi for her overload or Tali for her combat drone.
I don't think I have a real favorite. I really like Thane as a character. Zaeed fights like hell. I mostly mix them up depending on the situation.

Mass Effect 1: Either Wrex and Tali, or Garrus and Wrex. The former's for male playthroughs; the latter, for female playthroughs.
Mass Effect 2: Garrus and Miranda for the majority of the game; Miranda's passive ability comes in handy 99% of the time and with her and Garrus's double Overloads and her Warp, we can together handle pretty much any situation.
Towards the endgame, though, when I acquired Legion, I either went with Legion/Garrus from then on out for female playthroughs, or Legion/Tali for male playthroughs. Legion is awesome.

15 percent is nothing compared to 100 in adrenaline rush. I never experienced a significant difference .
But you make the assumption that a) the world revolves around you, or
For example, the 15% +damage is huge on higher difficulties if one were to play as a sniper, like an Infiltrator sniper that does not have adrenaline rush. That 15% can be the difference between a Collector being one-shotted by the Widow or that Collector having 10% of his life left.
Tell me you want to waste another Widow bullet on that same Collector. Sure, you can switch weapon, use Incinerate/squad powers, whatever, but it's infinitely better to kill Collectors dead in one-shot. A dead collector can't shoot at you, or turn into Harbinger.
