THIS DID NOT HAPPEN final mission Omega-4 Relay (SPOILER WARNING)

I have recently completed the Omega-4 Relay mission. None of my teammates died, and Joker said there were no casualties, but when I checked my team status I found that Tali was dead. I do not know what had happened, but I assigned her to the tech mission and she everyone survived (the door closed and I also assigned Miranda Team 2 leader-no casualties). Next, I chose Wrex as the escort and Samara as the escort for my team through the swarms. Miranda lead the others again. After this part, I left the rest of my team to defend the door while I brought Garrus and Mordin with me to the final boss. There were no casualties, as Joker also said, and Tali was still alive when I chose my squad mates for the final battle. Is this a glitch or is this supposed to happen???

Joker doesn't say there are no casualties, he says all survivors are on board. Based on what you've said about "Wrex" (I assume you mean Grunt) and Garrus, Tali actually should have died. If you'd like to know more about how everything during the suicide mission works, you can check out this thread on the campaign forum.

But I think mostly where your confusion is stemming from is that Joker doesn't say there's no casualties, simply all survivors are on board.

That's what that link I gave you would explain, but basically when you leave for the final boss fight, the people you leave behind for the defense will affect who is going to live or die. You sent Grunt away as an escort and took Garrus with you to the final fight. If you had left Mordin behind, he would have died. Since you took Mordin with you, Tali then dies. This is because she is considered a weak defender. Grunt and Garrus are two of the best defenders (along with Zaeed) and leaving them to defend that position is critical.

My first playthrough was the same, I lost Mordin for no apparent reason. I had sent Garrus I think to escort the crew and taken Zaeed and Grunt to the final fight. I usually send the weakest person back with the crew now to make sure they make it back okay, and no problems since then.

ok, I continued at my save point and i took Tali instead of Garrus and kept Mordin with me. It worked!!! ( and i got the achievement) Thanks for your suggestions. I wonder what will happen in Mass Effect 3?
