I've been noticing this a lot and its really causing problems on higher difficulties. I'll bring up the pause menu to switch a weapon and as soon as I do he "unlocks" from cover, becoming open to gunfire and getting me killed. It only happens with the weapon menu, not the powers menu. Is this a common issue?

it happened to me with the powers menu as well. but I think it only happens if I look around in a certain way, when the game is paused. In my case it is possible to see when this bug occurs when the game is still paused (a subtle change in viewpoint position - I know it, when I see it). If you are ready for it, you can avoid death. Also in my case it does not happen, if I do not look around while game is paused.
