This is utter bull****.
I can't start a new game, my old save character didn't go into combat mode, I reinstalled, got rid of everything, wasted my FCUKING TIME WITH THIS GAME, and I can't even get to play the God damned game I paid for to work.
Good ****ing job Bioware.

The same thing's happened with me. My characters stopped going into combat mode. I even loaded an old save, still didn't work. I uninstalled the game, reinstalled it - unable to load module. I'm re-downloading all the DLC to see if that's why it was unable to load module but I'm still not sure if my characters will be able to go into combat once that's finished

Solution: Go into your BioWare folder in your documents of wherever all your save file info is being kept - cut it and back it up somewhere, like your desktop. Copy and paste certain bits of it, like your character saves, your addins and the settings. That should fix it, worked for me

I've had the same problem. Haven't even been able to start a new game at all yet.
Question about your solution: Will we have to copy and paste the files after every log/save? or does once fix it for good?
I submited this problem to the EA support. And I'll post here with their answer, althought that may be a day or two.

I have the solution: delete "singeleplayer.cif" in dokuments/Bioware/Dragon Age/modules

Sounds like toolset installations...
1) Load toolset, goto tool > export > Empty Export Directories
2)
FIND:
Windows XP: My Documents\\BioWare\\Dragon Age\\modules
Windows Vista: Documents\\Bioware\\Dragon Age\\modules
Delete anything called single player
3)
FIND
Windows XP: My Documents\\BioWare\\Dragon Age\\modules\\single player\\override
Windows Vista: Documents\\BioWare\\Dragon Age\\modules\\single player\\override
Delete anything in this folder
4)
FIND:
Windows XP: My Documents\\BioWare\\Dragon Age\\packages\\core\\override
Windows Vista: Documents\\BioWare\\Dragon Age\\packages\\core\\override
Delete anything in this folder
