So, I'm an old man, and completely new to DLC. 
Anyway, I cannot seem to create an online account to work with this game.  I put in my email address and password while hooked up to xbox live and running this game, and nothing happens.  I get that intro screen forever, though without the "new game, load game" etc. options.  Finally, I just downloaded the dlc directly from xbox live.  But the game doesn't seem to recognize that it's been downloaded.  I talk to the guy offering the control rod, and he still wants me to download the thing.  (One thing making this difficult for me, possibly, is that I always play offline.  I never play on the xbox live account, and don't typically have the xbox even hooked up to the internet...it's a rare thing for me to have attempted this in the first place, and maybe I'll just say screw it and play what I can buy on a disc).  The downloadable content screen doesn't recognize that I've downloaded it.
I've enjoyed this game since it first came out, and never redeemed my code.  Now, I see that no code is necessary, even.  Maybe I should just be happy with the way it is, but somehow now that I've gone to all this work, I'm dissatisfied.  Crap.

To play DAO offline with DLC, you need to ensure that you have the necessary license(s) for the DLC that you're using. This requires an Xbox Live connection and logging in to the DA servers (not sure the second part is absolutely necessary, but I've never tried to install DLC without logging in to the DA servers). Fire up your box, connect to Xbox Live, launch the game, sign on to the DA servers, and go to Downloadable Content from the menu. You should be able to redeem codes here (which will change one or more items to Purchased) or spend points and download what you want. Downloading an item from this list gives you the license for that item. You will not have the offline license for any item that you do not download directly from this list. You can play DAO offline with any DLC for which you have successfully acquired the license.

You might also consider contacting EA Support and have them walk you through the process of setting up your DLC via their live chat. I can't guarantee that you'll get great results, but you would certainly get more immediate feedback for each step in the process. They should be able to determine if your login credentials are correct, for instance.
