 In the suicide mission, you get out of the Normandy and walk straight, but up the Collector Base. While this makes sense, you then enter the central chamber from the bottom, which would mean that the axis of entry changed. It's really not that big a deal, but kind of weird when you start to try and iron out how it could work.

On a more ridiculous note, in the first game, when your riding an elevator to the top of the Presidium Tower, you stop. You then leave the elevator, and start walking down, in the wrong direction.
