Cusp - dwarf swashbuckler (hour 28)
While in Amkethran Cusp saved a cleric being bullied by a monk and beat up some mercenaries for Saemon. On his way to Vongoethe he also saved the mayor's daughter and helped some smugglers - purchasing some boots and armour from them.
The lich proved immune to Cusp's traps. One set of protections allowed him to survive a first time trap, but the Runehammer proved fatal to him during the second.
On to Abazigal's lair. Draconis' human form was meleed before Cusp ran for it when a dragon arrived - quickly donning his shadow dragon armour and setting a couple of time traps before coming back into view. Abazigal did try a couple of breaths, but Cusp dodged both anyway. Once Abazigal reached one of the traps it seemed to take a long time for the trap to trigger (I suspect this was eventually done by an invisible stalker rather than Abazigal himself - dragons tend not to be where their icons actually appear on the screen), but eventually Cusp was allowed the opportunity for some free hits and his second WW of the timestop did the business.
Moving further into the lair Cusp explored a network of tunnels. Most things were meleed, though the Olhydra received the Gesen treatment. Bondari was given a job to do and was pleased to hand over an eyestalk wrapped in some bronze knickers.
Cusp has just freed an enslaved dragon and will be ready for the conflict with Abazigal at the start of the next session.
Cusp - swashbuckler L40, 160 HPs (incl. 13 from ioun stone, 12 from familiar), 1,464 kills

The best solution is to remove the drow illusion (the graphic effect of it only). I can vaguely remember reading something on this.
forums.gibberlings3.net/index.php
This issue is related to the method by which the illusionary drow
appearance of the party is implemented. AFAIK, the only way to fix this
is to install the "No Drow Avatars on Party In Underdark" component of
BG2 Tweaks. Here's a quote from the readme:
...
Adalon the Silver Dragon normally changes the party to look like drow
cosmetically. Unfortunately, the game engine has a number of bugs
related to this that cause the PCs to "stutter" and "stop walking" and
whatnot (especially if they have regeneration items). This disables the
cosmetic drow avatar bit only: the plot remains unchanged.

Time to settle some unfinished business. First up was to battle the Shadow Dragon in Umar Hills. This was much more tense than it should have been. Our very first move was to cast the berserker to draw out the death spell from Thaxxy - but I forgot that the death spell would draw agro, so Anomen foolishly threw a bullet at Thaxxy - we were completely unbuffed and no summons! Yikes!
We tried to get summons up, but we got buffeted and interrupted - however Vicky had the amulet of power and she did get off her Arial Servant. This allowed us to get more summons up next round and resist fear up. Next, we doomed x5 Thaxxy, lots of bolts of glory, then Anomen's finger of death finished it.

Next unfinished business was Firkraag. We battled to his lair, properly buffed and put 5 Arial Servants all around him. We did basically the same as Thaxxy, 5x doom, bolts of glory, slings and finger of death, oh and slay living - success once more!

We decided it was time to rescue Hanna, who was imprisoned in the Beholder's Lair. We recruited Imoen temporarily, and battled our way to the Beholder's Lair - we ran into a little problem though - two beholders were either still alive or re-spawned! We pulled out the stops, though not before 3 persons got petrified. We won, fortunately. We then used ring of earth control and earth staff to restore the petrified people. Imoen cast Freedom, and Hanna was freed from her prison. Hanna rejoined, and Imoen was sent back to the Coronet - the party is re-united once more!
Planar Sphere will likely be our next target.

Cusp entered Abazigal's cavern and killed a couple of frost salamanders before being spotted by the main man. He tried one time trap, but failed to kill the human form (didn't unequip the scarlet ninja-to in time) and then ran round for a while, killing the remaining frost salamanders en route (although one of them annoyingly wandered off far enough to trigger another time trap). With the salamanders out of the way Cusp set his last time trap and started taking pot-shots at Abazigal. I was rather surprised when the dragon form broke free as Abazigal's human form was only shown as injured at the time, but carried on with the battle.
Cusp hurriedly equipped some lightning protection and lured the dragon to the time trap. He started a WW with Carsomyr before switching briefly to Dragon Slayer and then decided Blackrazor would be more effective. Cusp did about 300 HPs during the time stop, which I thought might not quite have killed the dragon. However, he hit it a number of further times from continuing WWs, which clearly should have finished it off. I thought perhaps the rapid attacks had interrupted his dying speech, so Cusp ran away - which just resulted in Abazigal healing himself.
While Cusp was running round pondering what to do a greater earth elemental arrived on his side (a potential effect from Abazigal's tail slaps causing earthquakes?). Cusp tried setting a couple of traps during the distraction, but Abazigal just healed himself again. He then tried reading a spell immunity scroll, but a buffet interrupted that.
Cusp used Vhailor's Helm to create a simmy and then used the simmy to read a simulacrum scroll to try and bypass the summoning limit on them. I thought that should work and another simmy did indeed appear, only for the first one to disappear.
Next, he tried a time stop scroll with his last 2 WWs during that. Again it looked as though Abazigal should comfortably have been killed, but he remained at near death and refused to die. Getting desperate Cusp tried the nuclear option by summoning a mordy sword as distraction and attempting to imprison Abazigal, but that didn't work either.
Cusp was running round thinking about the best way to generate multiple summons at once to try a group attack (could have been done following another mordy sword distraction). However, he got caught with a ranged maze - you can see the distance apart they were in this screenshot and that was the same when the dragon started his spell.
Post the battle I checked Shadowkeeper to confirm that Abazigal should have 290 HPs in my installation. That was about what I estimated from reviewing progress in the battle from barely injured through to near death - it was just that he was soaking up huge amounts of damage while at near death. Hence the battle was clearly bugged. However, I don't feel any desire to go back and fight it again so Cusp will retire gracefully at this point.
Edit: the other thought I considered during that fight was to use a shapechange scroll and try and suck Abazigal to death with a mind flayer. In retrospect I should have tried that earlier as the stat drain might have bypassed whatever was preventing Abazigal from dying from HP damage.

@Aasim - Thanks! Yes, the team fights well, and now has some powerful spells to support us - took quite a while to get to this point. I'm concerned about +4 melee weapons in ToB though.

I can understand not wanting to redo the fight. I'd encourage you to keep Cusp's save for now, however. Take a walk; ride a bike; read a book; eat a pizza. After a few hours you may feel ready to resume Cusp's adventure.
Best,
A.

Jhor the Bleeder can also cause issues with dying dialogues.
Don't know if it's relevant for your case, however.
Imo, feel free to reload this - it's an engine issue probably.
@Corey
There will be plenty +4 (and higher) for you. 2 flails, 2 hammers, a staff, and 2 maces. Don't forget +4 bullets and energy blades as well.

While there +4 bullets and energy blades, both of which are of limited supply, there's still not enough melee weapons. Here's the math for you: Front-liners (2) dual wield - 4 weapons + 4 weapons for sword and shield guys=8 but you only listed 7..
Zetran and his band of Fighter/Clerics - FINAL BG 2 Update
Traveling with: Hanna, Melissa, Vicky, Paja, Anomen
The team battles in Bodhi's lair, with paladins and thieves helping us - the team did excellent, no trouble at all.

So Zetran's team battles in Suldanessar. The team plays carefully, and Zetran uses the staff of smiting here to good effect. One of the key battles was of course against Nizi. It took a long time, but the party prevailed without any deaths.

For the Tree of Life fight, Zetran used a pro-magic scroll and tanked Irenicus - the party didn't do too well, but they eventually pull through.

Zetran still had his pro-magic scroll, so used it in the Fear challenge for the beholders. It didn't last for Irenicus though. We did a ton of buffs, perhaps most important being invulnerability potions on everyone. Here's our prep if interested.
The party uses their impressive melee to burn down the demons, giving most party members their first ToB ability. Once the demons and Irenicus image was down, we summoned more friends, then the front-liners attacked with the summons and everyone else with their slings. Took a little bit, but we did huge damage and won the fight!

It's surprising that we had no chunks in BG 2 - possibly this is true due to our considerable self-buffing we are capable of.
Hanna - 100 HP, 322 kills, Bodhi
Melissa - 105 HP, 340 kills, Nizidramaniiyt
Paja - 96 HP, 112 kills, Unseeing Eye
Vicky - 90 HP, 150 kills, Ulitharid
Zetran - 100 HP, 217 kills, Balor
Anomen - 118 HP, 154 kills, Firkraag
It's amazing that Hanna got the 2nd best kill rate considering she was imprisoned for part of the time.

FYI: + 4 or greater melee weapons useable by clerics.
Clubs: Club of Detonation
Flails: Ages, Ice Star
Hammers: Crom, Runehammer
Maces: Storm Star, iMoD
Quarter staves: Staff of Rynn, Staff of the Ram
As for the +4 bullets, they are limited but there are a lot of them. The Sling of Seeking is my favorite weapon and I've never run out of +4s. Just use the +3s against enemies that don't require +4 to hit (the +3s will work on the vast majority of enemies you'll encounter).

Zetran has finished their Saradush quests. Gromnir battle was interesting - we buffed to the max, did one action, then put on AI - we won cleanly and easily! But problem was Melissan didn't appear - so had to do it again - we did even better the 2nd time, even though I did the exact same actions, except put an Arial Servant by the enemies by the stairs.
We intend to hit Watcher's Keep next, we really need the ammo belt.

Zetran and his band of fighter/clerics - Update 13
The team has successfully obtained the two hearts from Yaga's Temple. Our most difficult encounter to date was the shambling mounds assisting Nylee. Our weapons were useless, so we had to all our magic offensive spells, as well as summons that don't use crushing damage. Took quite a while but we eventually prevailed.
For Yaga, we summoned what summons we could and did 3 round of buffs and commenced the fight. We were on AI for most of the fight, but once Yaga returned I turned off AI and concentrated fire - he fell almost immediately.
Oasis is our next target.

Stiel, halfling assassin (Grond0) & Andal, halfling bounty hunter (Gate70)
Our thief heroes continued on their quest to
Andal was taken down to 9 HP by one fell cat in the lower temple and then to 1 HP by another in the upper area. With them all dead, along with Berenn troubles seemed to be over. However, alag hit from a giant eventually proved too much and put Andal down for his 8th death - good job he's got feline blood!
One curiosity noted at the temple was this cheeky fire giant - merrily taking healing potions during a time stop. I also belatedly noticed this backstab - a new record I think for Stiel.
The duo finished looting the temple and have just arrived back at the Temple of Mir.
Stats:
Stiel, assassin L34, 147 HPs (incl. 12 from ioun stone, 12 from familiar), 837 kills
Andal, bounty hunter L34, 112 HPs, 676 kills, 8 deaths

Zetran's team managed to battle their way to Sendai herself. We did maximum buffs of course, perhaps most important being fire resistance potion on Zetran himself. Most of the battle was AI, believe it or not. I did turn off the AI for RoR heals (a few not many). We did heals, a few spells, mostly summons and refreshing as they were slain. No deaths, but Sendai does in fact fall.
We are running out of the good ammo, but we'll have to make do. Draconis is our next target.

Sorry of your loss, Corey... That's a bit weird for clerics do be affected by fear with Remove Fear being level 1 spell... Great run nonetheless!
We fought the mage form with summons, so buffs were irrelevant. But the problem was the mage form died very close to the entrance, which is where we were - thus we had no chance to buff, we did try to get a resist fear up but the wing buffet interrupted our spells and we never got a chance to recover after that.

Some news of the Avatars. Krimlor tried his luck with Aesgareth and wager experience vs Spectral Brand. Strife vs Construct (drained strength and lost some exp). Tried again: Strength vs Defiance > so won Spectral Brand in the end at the cost of some exp. Last bet was a way out and we let Aesgareth go - he is quite a nice demon after all.
Talked calmly with Carston so got just lightning strike and no mind flayers. The main area was uneventful though when Krimlor turned the Shadows and Shadow Fiends there he destroyed his own Skelly Warriors as well...oops:?
We did not tempted fate vs improved Gith or Mind Flayers so that areas were mostly soloed by couple of Planetars though Pot had to intervene with ADHW when Gith stunned Planetar with Mind Blast and even killed Anti-Palladin with Firetooth! We had a plesant chat with Saladrex and Krimlor eliminated Demi-Lich buffed with Pro vs Magic scroll.
Improved Mind Flayers are lot more troublesome and we needed two Planetars, one Deva and a pair of Mordy Swords to clear their lair.
Rock and Garock are pushovers and after some fiddling with Machine of Lum we descended into the final level.
to be continued...
