Apparently you can lose at least one squadmate no matter what you've done, just some weird odds thing, most of the time you get by without anything happening but sometimes that doesn't happen.

I took Tali and Garrus through the whole run. No one died except Grunt and Miranda. Grunt was my mistake - made him the diversion team leader. Miranda was disloyal, though, and I hardly cared less.

Your Hold the Line score was too low. Since Mordin wasn't there to die, the next person in the list bit the bullet... Tali...

You can take Garrus, Grunt, or Zaeed to the final battle without anyone dying. You just need to leave at least two of those guys there. I've taken Tali and Garrus to the final battle everytime and Mordin only died after I got Zaeed killed as an experiment.

Just a thought - I assume everyone has upgraded all the weapon and armour options for the whole crew? Does anyone know if that makes a difference? It should. If so, then it's a three way thing on survival - loyalty, upgrades, and right choices for specialist roles at the end. I've been through four times, and only once lost Miranda - but I knew that was a gimme as I didn't have enough persuasion points when I was settling her fight with Jack. Gee - didn't know Kasumi was tech - isn't she assassin? Maybe I should learn to read the box, so to speak. Does seem like whoever you take with you at the end, provided they're loyal, they'll survive. Would be interesting to get up a board of everyone's mix and matches.

cheers

Whether or not you have Jack's biotic upgrade will not affect her ability during the suicide run, I tested that. As far as I know, none of the upgrades aside from the ones for the Normandy have a direct affect on the suicide run.

If you ever wanted to know more specifics about the suicide run, there's always this post that's stickied at the top of the forum.

Nope,Mordin lived as well...

I just finished my 5th play(on hardcore with Geth pulse rifles) and used Mordin as escort this time around and everyone survived...
and Kelly is still dancing at my armor locker....:P

Mordin wouldn't have died if you sent him as an escort and kept Grunt. But I bet he died when you sent Grunt as an escort. Try it. Reload that 5th play and send Grunt as your escort and take Kasumi and Zaeed to the last battle. I bet you Mordin dies.

The only time any of my crew or squadmates have died was in my first play through and Jack wasn't loyal...
but I have used the same assignments since then except for the last time where I swapped out Grunt for Mordin and everyone has lived 4 out of five times....
I am going to start an Insanity playthrough with a different class and maybe a female Shepard and I may try a different rotation in the final mission....depends on how much of a b**** I decide my femshep is going to be....LOL

If you don't want to lose anyone or risk it, take Mordin, Tali, or Kasumi (a variation of those 3) and you will suddenly have two 1 pointers missing from your Hold The Line and all your 4 pointers. Everyone survives... happy happy joy joy.

Nah, I'm not going to buy it. If you sent Grunt as an escort and took Zaeed with you to the final boss, Mordin died.

It's actually quite clever all the different outcomes we can get from the Suicide Mission.

My first playthrough Jack died since she was not loyal ( I did not have enough paragon points to stop the confrontation between Miranda and Jack without taking someones side and seeing as how Miri was my LI and I didn't like Jack... ).

On my second playthrough Mordin kept dieing even though he was loyal. It took me 2 or so tries ( and then a quick search on the net ) to figure out why he kept dieing. Apparently he isn't a strong enough front line fighter to guard the door so there is always a very high chance that he may die there, so now I send him back the Normandy to escort the surrving crew members. In your case it probably doesn't hep that you are taking Grunt to fight the last boss, taking him ( and guys like Garrus ) with you to fight the last boss means that you are removing alot of muscle from the guard duty and that leaves the weaker charcters very vulnerable.

I still can't believe I'm going to have to play the final mission *again* because while the second time around I managed to save Mordin, Tali died like a dog for no reason! This time I'm taking them with me, it seems to be the general consensus. But it sucks that I did everything right, they were never in any position of leadership to avoid premature death, and the camera still hovered over Tali's dead body at the end. Again, for no reason.

As a summary, and to repeat what others have said, as long as you leave Grunt and Garrus with whoever else to hold the line, anyone who is loyal will survive. Leaving only one should be OK too, but maybe there are some combinations that might cause random deaths of the weakest team members, eg Tali, Mordin.

Does anyone else think that the individual weapon biotic tech and armour upgrades should have an effect? Yes, I know this is making things an order of magnitude more complex, but computers are made for algorithms! Opps- forgot people have to write them first... In addition the balance of the powers of each team member should make a difference. But fact is, if this is really to mimick real life, then no matter how well you plan and set up for events, sometimes s**t just happens. Hmmm - I would find this hard to take as intrinsically as this is a game, I expect (consciously or subconsciously) that if I do things right, then I will get the maximum reward. Like I said, anyone's real life like this? Although if there was some story attached to it eg after battle, some cinematic or even just a 'battlefield report' as to how incredibly unlucky so and so was to eat dirt, but oh well, life just sucks sometimes, that might be enough.

cheers

If I want specific members to DIE, how do I do that? ( I want Kasumi, Zaeed and Jacob to die. )

Have Jacob do the vent and don't do Kasumi and Zaeed's loyalty mission's then take them to fight the human reaper

Also this might help
