One detail I would like to see adressed in DA2 is this business of comrades being killed and then just getting up again once the battle is over. I realise this is a gaming issue, but it jars narratively because of course in real life it wouldn't happen.
I propose therefore an adjustment to this idea, where when close to death characters fall to the floor and feign death, effectively, instead. I appreciate there is still a matter of unlikeness about the notion, but it's a bit more 'real world' than the current formula, and as best as I understand things, would not be difficult to implement even with the current build of DA.
Thus, it would appear that your characters are faking their own deaths when injured badly, which is more believable than them somehow getting up again after death.
I note a difficulty with the idea, in that you may not be playing as your own character but might have taken control of another, so what happens when all four of you are feigning death? Narratively, it's a bit of a tough one, but on the whole I feel it can be worked out better than the current, well-used system.

A fallen companion is never dead -- they're just unconscious. The injury a 'fallen' companion gets is the evidence.

No, it's the face of death that you see on your screen. Your characters are dead, the injury is just an after-the-fact rationalisation of that.

As it stands, it reads more like a sort of penalty point against you for your characters being down, where a minor adjustment would make the idea more rational and less 'gamey'.
