I would like to point out that not only the most vile people in all of creation are going to engage in incest. Sure it is not something that western society likes to even think over much about, but under the right circumstances one could see a loving - though still dysfunctional - relationship manifesting between siblings. I once played a game called "Kana" where the incest storyline was an intensely emotional issue about not only inappropriate closeness between siblings, but engaging your sister in an experience she'd never have without that. It is considered one of the most intense and emotionally powerful games of its type and you will find virtually no one who has played the game that won't defend the value of it in the story. (predictably, though, people whom have not played the game almost universally fail to understand how it could be done well)

That people behave by the norms of our society is...odd. It wasn't that way in real life, and it shouldn't be that wya in the game.
Of course, they won't actually allow incest, and they probably shouldn't because then we'd have some idiotic investigation by Congress into video games, but for "authenticity", if you will, to the setting then it would not be out of place.

Only example of incest in fantasy in a not negative is in Discworld. In ASoI&F it´s not a valid example. One character is Stupid Evil and the other bitter to the core. When Jaime gets better the relation kind of stops.

And what, pray tell, do you think the norms of Medieval society were, that were so different from our society in real life? I will tell you - the norms of medieval society were determined by the Christian Church, and public violations had wide-ranging consequences. For Catholics, I'm not sure about second cousins, but first cousins could only be allowed with a dispensation - anything closer was a violation of natural law. For Orthodox, third cousin was the closest officially allowed. In either case, a powerful figure violating these norms would be seen as offending God, and any major disaster in the realm would be blamed on the violation.

The Dragon royal family perpetuate through incest. All of them. It's not only Jaime and Cersei.
