Im not really one to have multiple playthroughs of games. I like to play games straight away on the toughest setting. I noticed that harcore and insanity are greyed out to start. I unlocked Hardcore with a hack. Im struggling to see the achievement to unlock insanity. Any idea's? I will also use my character that created in ME1 and import over to ME2. Will these hacks affect the import?

To unlock Insanity, you must complete one playthrough on Hardcore without changing the difficulty during the game.

If you were able to unlock Hardcore with a hack (a way ta gain achievements, I presume?) I would presume you could do the same with the Insanity achievement.
Trust me though, you're not missing out on a lot if you aren't able to do Insanity. Longer/more inane fights != tougher ones. ME2's Insanity does a much better job of being truly difficult by making you more vulnerable. But ME1's Insanity just means your enemies have lots of damage protection so that you have to use Warp and hold down the Shoot button over them longer.
