How about you'll get approval with your companions also when you use them in fights? Of course not every fight, that would only lead to farming, but for example whenever you defete a boss you get a small amount of approval-points with the party you are using to reflect that they trust eachother in a fight or that their teamwork have improved. And perhaps also uniqe dialog when talking about the previous fight back at camp?

What if you use them as meatshields and they come back with broken bones, injuries, etc. A system that rewards combat use should probably be complex enough to reward how they're used... and that probably makes implementation too hard.

Maybe you could earn one or two approval points for saving an ally from falling in battle, but nothing beyond that. As you said, people would abuse this.

Well, maybe you can lost approval when you never use a companion, since the game takes place in ten years and they should not be so happy to waste their time waiting for so many time in the camp.
Other than that, I'd remove the non-plot gifts, or at least greatly re-balance their effects, since in Origins it was too easy set up approval with a companion just by offering gifts to him/her.
