Also, Flemeth swoops down, grabbing a Darkspawn in her talons and then turns to a human from Flemeth and says "Well, well, who have we here?"

I'd personally classify it as both... The Chasind people called her Flemeth, or the Witch of the Wilds. And as she grew old, she had to take the form of her daughter as we all know. It's highly doubtful that there is more than one Flemeth, at least at a time.Grommash94 wrote...
It has been a discussion that Flemeth is a title, not necessarily a specific person. So, yes, it could very well be Morrigan.
It also depends when the events in the screenshots take place.
But, if it IS Morrigan, and the "Flemeth" you either killed or let live hasn't found her yet, it is possible that Morrigan decided to become a Witch of the Wilds and take on the name, Flemeth.
Or, of course, Flemeth found her and took over her body. Though, Morrigan has the Grimoire...

The Baroness manifests herself in Blackmarsh after you escape the Fade - may I add, without needing to reanimate a corpse or enter a living body. Flemeth may have done something similar.
