I would say Ocarina of Time had one of the worst endings given how amazing the whole game was, and then it just concludes with a big kombuya circle of the game's population. That was very disappointing for me as a kid.

The base of the entire story is the assassins, it's called "assassins creed", not "animus creed". In dragon age, the base of the story is the dragon age, not the darkspawn.

Another bad game ending was the finale to Rayman. After a lengthy but fun platformer and a final battle with Mr. Dark you're given a short video clip wherein the camera pans over a fairytale landscape while an off screen voice congratulates Rayman. <_<

Another bad ending... the "neutral" ending in Jade Empire. In exchange for surrendering to the antagonist your character's legend lives on. A statue is erected in her/his honor and worshiped by the pod people of a new Jade Empire. It's an apt ending if your character is a quitter, but it still kind of sucks.
The evil ending to Arcanum: your character teams up with the big bad and destroys the world. He turns on your character and after an epic battle is narrowly defeated. Since all life has been extinguished and your PC has become effectively immortal, s/he wanders around an empty world until the end of time. All this is related to the player through voiced text over a bland background.
The ending to Axelay. This game was a *phenomenal* shooter in its day. The ending recycles an image from the opening cut scene and redraws it slightly so the pilot's visor is cracked. You get a congratulatory message for saving the Earth and credits roll. Considering how hard Axelay was, it was total let down.

For one, with so many endings to DAO I wonder what ones are considered "so bad to be worst" outside of having fun mocking BioWare on their own forum to their (you'd expect) fans...
I only did Awakening once so far, but I found the game to be just fine and don't remember having an Inception moment, even on reflection, with the game's ending.
I'd say Leisure Suit Larry in the Land of the Lounge Lizards, the first one, because after you "score" it's a black screen with a few very lame fireworks and a "the end" caption - but then Al Lowe, the game's creator, walks out on screen and says something like "what did you expect?" So, on second thought, best ending ever.
I can list great endings - like Breakdown or ME1 - but I can't think of bad ones. Hrm. You'd think they'd stick out. Maybe I pick my games very carefully (in fact, I know I do.)
I can think of unsatisfying endings, in which I wanted to know more (Starcraft: Broodwar, for example.)
Nope, I can't think of any bad endings. I know they are out there - and I've probably played games with bad endings, but I'm thinking that I didn't finish most of those games.
