Ok so first for the achievement glitch.

When ever I load it up it tells me I unlocked an achievement and essentially loggs me off of xbl(Cant view profile, cant view achievements ect.)

Second is...well just look....

Please help for both and by the way for the achievement thing, it wont let me unlock any more achievements when I start up the game.
