It would be more realistic to keep helmet on but since this is a game and it doesn't matter much, I usually do not use it. However, I don't think that it looks bad on my Shep. Even the opposite since the hair in ME isn't that great. It does add realism - I keep helmet on for Sheploo and I would always wear helmet in army (even when it would be optional; that's rare, though).Interactive Civilian wrote...

Realistically, you would remove your helmet for all talking scenes and wear it in battle. However, since ME2 has no toggle option, you are forced to just leave it off.
In ME1, I guess I could technically toggle the helmet constantly back and forth...but I'm lazy. So I just leave it off and pretend I put it back on in combat

Assuming all the spoilers we've been reading are true, I wouldn't bother.
Part of me is happy though. Now I won't have to kill off or otherwise character assassinate Bishop (Caroline's out-of-game LI) in fanfic. Because I might have been tempted.
