It's a mod.  You have to add it in by editing the ini

ya know, i've been playing da:o again (after mostly ignoring it after several abandoned playthroughs) and it's really weird to me how rarely i ever see my warden. in ME, you see shep ALL the time. fighting, obviously, but the cutscenes show her off so much. with the warden though, it's like i never see her - they rarely ever show the PC. it's really strange, because even back in jade empire and kotor you saw your character all the dang time. it really fostered this fondness for your character. but in da:o, it's like even the game doesn't want to look at the warden. it's really strange.

A new character design concept I've been playing around with, Erica. I went with the sort of vaguely hispanic look by using a modified version of Ashley's face texture. I was thinking about making her a Sole Survivor, there's a certain sadness behind her eyes . . .
