Jakarta got hers on Elysium too - she was grappling with a krogan and someone set off a tech mine right next to them, catching them both with a fair bit of shrapnel. They didn't really have the time or medical facilities to heal it properly in the middle of the Blitz, so she just slapped some medigel on it and kept going. She's not proud of them, exactly, but she's not particularly bothered by them either; to her cosmetic surgery to remove them would be as extraneous and purposeless as breast augmentation. She's a soldier, scars happen, its nothing to get worked up about either way. That said, she'd be pretty vexed to wake up without them, because it would be like waking up with a different nose. They really do a lot to define the way she looks and she wouldn't recognize herself in the mirror without them.
Which is why I TexMod them into her ME2 game; I can actually see Miranda and Wilson fixing small scars (or rather, choosing not to restore them, since I don't imagine Shep actually had much skin left at all), but if the idea was to bring back Shepard for her celebrity and public icon status as much as her particular talents, then scars as dramatic as Jakarta's would pretty much have to be kept.
