Given what Shepard has been through I think the word damaged could describe a lot of Shepards, I know Kate has gone through phases where it would apply both mentally and physically and it's something I've been exploring in my attempts at fic writing.
Edit - I know when playing ME2 it struck me as unbelieveable that Shepard could die and be ressurected and not having an issues associated with the whole process.
That's why I have 2 versions of Tess (although I intend on re-doing her... she's currently a spacer, sole survivor, and infiltrator but I think it would fit better if she was a spacer, war hero, and sentinel... anyway).
She's the only Shepard I've role-played to be actively religious, and it defined her life and gave her a code to live by. Tess doesn't remember anything about her time spent dead. If there was an afterlife, she certainly doesn't remember any of it. In one version, Tess just doesn't believe that Cerberus is capable of something like resurrection and chooses to believe she was in a coma or very near death.
In the other, Tess has a crisis of faith. She just can't reconcile her resurrection with her religious beliefs and becomes extremely disillusioned. She goes from being an uber-Paragon in ME1 to a fairly cynical Paragade in ME2. How do you live when you can't trust the rules you used to live by anymore?
I think I posted her yesterday, but a pic for reference:
