Yep, my first playthrough I didn't lose a single person. Mostly because when I play games I methodically do every sidequest available before tackling story quests. Did it in ME1 as well. So, when I went to do the derelict reaper I'd already finished like every loyalty mission but Legion's. So, I went straight through the relay when the crew was taken. I also followed the advice of Miranda... picked Legion as tech expert, made her fire team leader both times, used Jack as biotic, sent Mordin back (because in my brain I thought "he's a doctor so he can take care of the crew's injuries"). I took my usual squad of Thane and Garrus to take out the baby reaper and everyone got out alive.
Moira is my only Shepard who didn't "stay loyal", but that's because she believes Kaidan walked away from her and the relationship is over. To put it plainly, she's angry with him. Moira isn't very experienced with intimate relationships. She doesn't realize he was shocked and acting on his emotions.
Moira also changed quite a bit after her resurrection. She used to be less of a Renegade, but now she doesn't have any qualms about doing what is necessary to save the galaxy. She's more ruthless, colder and less patient with people who get in her way or waste her time. In essence, her and Kaidan would have most likely drifted apart... even if they were back together.
She decided it was best to move on after what happened on Horizon. In my head canon she finds out that Garrus has always had feelings for her. They grew much closer after reuniting on Omega and during the mission to stop the Collectors. Moira realized that she loved him also, and things went from there. I think she'd still want to be friends with Kaidan, but Garrus is in her heart now.
