UPDATE: I am giving out my system information to make this thread more useful for the developers (from the Mass Effect Configuration Utility):

Quest related glitches:
-In the first mission where you have to escape the Cerberus facility the left shift menu indicates you having 3 weapons (towards the end of the mission). The Predator pistol, the Grenade Launcher and an unselectable
nameless weapon with an icon "icon missing".
-At the end of Mordin's loyalty quest if you choose to stay sometimes you can not end the mission by pressing and holding F button. Moreover a selectable rectangle appears in place of Maelon (it says Maelon). Even if you assign another key to melee attacks, the dialog box still tells you to press F. If you press and hold the new key you assigned you can then end the mission. UPDATE: Something similar happened to me in the mission in which you disable a nuclear warhead. After choosing whether to save the city or the space station I was not able to finish the mission in any way.
-In Jack's loyalty quest there exists an unreachable door in the second floor in the large room where you fight the Vorcha (in the one where they attack from the windows). This door opens and closes when you approach
it despite you being on the ground floor.
Glitches related to user controls and getting stuck:
-When using the M-44 Hammerhead to harvest yellow resource circles, the game tells you to hold down right mouse button. However doing so nothing happens; you have to press and hold another key (that was 'V' for me) to harvest resources.
-Party members sometimes get stuck and do not respond to power use and move orders when they do move behind cover. In that state they do not acknowledge orders by voice.
-Party members do not respond to power use orders (from the Left Shift menu) despite acknowledging and responding to move orders (usually occurs when they are behind cover). After moving around a little they begin
responding to power use orders again.
-You sometimes can not acquire targets despite them being all in open and you having a clear line of sight to them (from the Left Shift menu).
-Shepard faces some other direction than forward and moves towards that direction when you press the forward key (W). This happens extremely rarely when you move around and press left shift at the same time. Bringing
up the main menu and then resuming your game fixes Shepards's orientation.
-Shepard slides instead of walking while in the middle of an omni-tool scan animation.
-Shepard sometimes gets stuck when he runs into tables or on inclined surfaces (being instantly appearing on them and then being unable to move). He gets stuck in various other places too (randomly).
-When you use a power while holding a sniper rifle (from the Left Shift menu), the rifle zooms in making it really hard to use powers wielding a sniper rifle.
-You can not skip the Illium landing and leaving video like you do with other planets (intentional ?)
User interface glitches:
-Sometimes a black rectangle appears behind the choices in the dialog wheel (it appears in subtitles too).
-Incisor Sniper Rifle does not have the correct serial numbers after upgrades.
-Some powers when fully evolved have reduced durations for party members (like the Unstable Warp). Numbers boxes that show power's statistics at various levels display the incorrect (upgraded) Recharge Time for
levels 1,2 and 3 (intentional ?).
-You can highlight and select your remaining squad points which is displayed in the upper left of your screen (intentional?)
-Typo: Group Dominate: Increases Dominate power against all multiple organic targets near impact point.
Also the way it puts it is confusing in my oppinion. Should have been something like: Dominate effects all organic targets near impact point.
Glitches in the Galaxy Map:
-If you move away from the fuel depot before you completely refill your supply of fuel, you get stuck in a sound loop of fuel being loaded into your ship. The loop ends when you land a planet or leave the galaxy map.
-Sometimes when you leave a planet close-up and return to the solar system view (the ones you can visit planets and moons) you become unable to move. You can enter the orbit again however and after leaving the close-up once again you are able to move your ship again.
These are the ones that I found. Please add your findings.
