First off, let me say I loved the pre-order items that I got with DA:O, but my only problem was with how they were implemented. In my opinion, they shouldn't have been available from the start.
One of the main draws of RPG's is character progression, transforming a weak nobody into a demi-god is fun. The 2 ways that happens is with stat progression via level ups, and loot upgrades. Having such amazing loot so early completely undermined the feeling of character progression via loot until 3/4 through the game.
For DA 2, I think it would be better if pre-order and DLC items are either attached to quests as extra rewards, or placed in treasure chests in areas appropriate to the power of the item. That way, the gameplay balance of getting better gear slowly over the
course of the game remains intact, and the DLC gear
is still awesome and powerful and will still encourage people to
pre-order. The only other way I see to maintain a balanced item progression is to release weak DLC items, which probably not many people would like.
A certain wild-west sandbox game recently (Think it was called Scarlet Deceased Salvation or something similar) that had a DLC outfit required you to earn it in-game by completing challenges. When you do eventually get it, it feels like an appropriate upgrade, unlike the Blood Dragon armor etc., which kinda felt like cheating.
Also, inb4 "If you don't like it don't use it" - I like making the strongest possible character I can, if good loot's available then I'll use it. I don't want to have to restrict myself from using loot. I shouldn't need to self-enforce gameplay balance, that's the game designers job.
Please implement DLC items better than in DA:O

I kind of agree, I had a tendency to do warden's keep first every run through just for the storage chest, and I'd end up with this amazingly good set of armour to boot.
Then I'd want shale, cause shale is cool, so I'd have some pretty nice stuff from there too, my latest playthrough I didn't do return to ostagar till quite late on and I was glad for it because that juggernaught armour is hard to get, it was nice to actually USE it for once.
As for blood dragon: haha I've never used it, it looks TERRIBLE.
The only issue I see with staggering item DLC is that people might not get what they've paid for. (I remember some people complained because Alistair could leave with your warden commander armour ;p)
People tend to want what they've paid for now, not after they've completed some challenges, if this DLC is a side quest then that's fine, you're there to do the quest, if it's specifically items then, you've kind of paid to have those items, and you are a sucker ;p

I felt a touch suckered by how horrible blood dragon looked, I never used it due to just how bad it looked. I think you're probably part right though, this honestly depends on the opinion of the player, but I've found gamers to have a sense of entitlement, so many of us will want what we paid for RIGHT NOW!

Well DLC for DAO and Awake is a borken buggy mess that Bioware is blaming on Sony/Microsoft.
At least they said they have found the root cause and *promise* to avoid in the future.
