There is a graphical glitch that affects every single one of a certain type of item in Awakening that wasn't present before in Origins...it is present as of both patches 1.03 and 1.04.

The bug:
Every helmet that shares the same graphical texture as Helm of Hirol's Defense (an Awakening-only item) ends up with the same colors as Helm of Hirol's Defense.  There is no exception to this, which affects helmets of this type either imported from Origins (DLC or base game) and items native to Awakening.  This issue is only present in the Awakening campaign.  Helm of Hirol's Defense appears with the following colors: silver faceplate, gold dome, and white plume, and all helmets in Awakening that have the same texture will share this color scheme instead of using their correct color scheme.  This bug has been observed with the following helmets:

Items affected imported from Origins DLC:
Cailan's Helm (from RtO DLC)

Items affected imported from Origins base game:
Soldier's Heavy Helm (red steel)
Soldier's Heavy Helm (silverite)
Soldier's Heavy Helm (dragonbone)
Note: Didn't bother testing this with helmets of the other 4 material types, but I presume the results will be identical.

Items affected from Awakening:
Soldier's Heavy Helm (dragonbone)
Soldier's Heavy Helm (white steel)
Soldier's Heavy Helm (volcanic aurum)

This is especially annoying, of course, since now all helmets of this type look identical to each other.  Cailan's Helm, for example, just looks weird without the purple plume.  Additionally, trying to make matching armor sets is impossible when all the helmets aren't colored the way they're supposed to be (dragonbone Soldier's Heavy Helm is supposed to be red, for example).

Note that there are 2 similar-looking helmets that you will find that will appear to be exceptions, but are actually not.  Juggernaut Helm (from Origins) and Grey Warden Plate Helm (Orlesian Wardens will start Awakening with this) are 2 such examples, since they both appear fine in Awakening and don't suffer from the bug described above.  However, this is because while they look similar to the helmets above, they are actually a different texture and thus are not (and should not) be affected by this bug.

I have tested this bug with all of the example helmets above, and every Awakening campaign I start or savegame I load are affected by it.  Additionally, I've viewed these items on a friend's computer who also has DA:O, Awakening, and all DLC's installed, and the results there are the same.  This bug is entirely reproduceable.

Specs:
OS: Windows XP Professional x64 Edition, Service Pack 2
Processor: Intel Core 2 Quad, Q6600 2.4GHz
Memory: 4GB RAM
Video: NVidia GeForce GTX 260, 896MB VRAM
Video driver: Release 256 Version 258.96 (released 7/19/2010)
Awakening and all DLC's (up to Leliana's Song) installed, patched to 1.04 (though bug was also observed to be present as of 1.03).

To anyone on Bioware staff: Any chance this bug gets addressed in an upcoming patch?  It's rendered my Awakening campaign unplayable because my character looks stupid when the helmets that match my preferred armors look mismatched and silly due to a bug.

You have ignored the more important aspect (inasmuch as no one else has reported any such error, particularly).  What is the graphics environment that you are requiring the expansion to run in?

Video (GPU) Manufacturer: ATI / Nvidia (only those two are supported, PERIOD, so please answer which of them you have)
Video Card Model: (examples) Radeon HD 5670 / Geforce GT 240
Video Card Driver Dates and Version: Catalyst ?.? / Nforce ??.??
. . (please use the numbers, not the word "latest")
Video Card onboard RAM: ? ? ? MBs

Operating Ssystem, and Service Pack Number

Thanks for the heads-up.  I've updated my original post with the relevant information.

I thought I'd include some picture samples of this bug.

Here is how Helm of Hirol's Defense appears in Awakening (all helmets that share its texture end up looking identical to this in Awakening):

Here are how some of the helmets I described in my first post supposed to look like (all the screenshots below were taken in Origins because they don't appear correctly in Awakening):

Cailan's Helm

Soldier's Heavy Helm (dragonbone material)

Soldier's Heavy Helm (silverite material)

Hope this helps in fixing this bug.

I wonder if they just recolored the texture files in Awakening that shared the same name with those in Origins. I haven't tried to import any of those items, so I couldn't tell you whether the same thing happens to me or not.

But, anyway, I'm pretty sure this has absolutely nothing to do with your graphics card, and even less to do with your specific computer configuration ;-)

Agreed, I'm sure it has absolutely nothing to do with my graphics/computer configuration, but the info never hurts.

This bug is actually very easy to test/reproduce.  Playing through RtO will get you Cailan's Helm, which you can just import into Awakening (doesn't matter what level your character is) and see that its color changes from Origins to Awakening.  If you don't have the RtO DLC, you can always test it with Soldier's Heavy Helm of any material type by purchasing from vendors.  Off the top of my head, I know Herren in Denerim sells it, as well as Owen the blacksmith in Redcliffe...the material (and appearance) will vary depending on your character's level upon purchase.  Take a screenshot of the helmet after you buy it, and compare it to how it appears in Awakening.  It's a little harder to test the helmets that are native to Awakening (because in Awakening is when they get bugged), but it's very easy to notice something is very wrong when you purchase Soldier's Heavy Helm from Herren in Vigil's Keep and notice that they all appear identical to Helm of Hirol's Defense no matter what material type they are.

I'll see if I have a character who's already completed RTO I can quickly import later tonight. You've piqued my curiosity.

Edit: I purchased Soldier's Heavy Helm in Redcliffe, and here are my results (apology for the crappy screenshot quality).  They probably did recolor the files.

Thanks for the additional testing and confirmation of this problem.  From my untrained eye, I would guess that the problem is an assignment issue.  I find it far likelier that someone accidentally assigned the colors of Helm of Hirol's Defense to apply to the texture itself (so that all helmets suffer from the problem) as opposed to every helmet of this texture accidentally recolored to look like Helm of Hirol's Defense.  Far likelier to have one mistake occur than many mistakes of the same type, you know?

Update: This bug is also present in the Golems of Amgarrak DLC. My (uneducated) guess is that it's because it shares the same resources as Awakening.
