Nate's Theme
From Uncharted/Uncharted 2 Among Thieves. The latter being by far the best action/adventure game I've ever played. And with Civlization IV and Baldur's Gate series makes the top 3 games I've played of all time. Too hard to choose amongst these three for me as they're different experiences.

A number of themes mentioned I have to agree with wholeheartedly: The opening to Civ IV is truly astounding. Some of the background music to accomplishments in Civ II were also quite strong and evocative of their era.
However, being an old school gamer and a game reviewer for years (for games for the Apple II and the Mac, if that perhaps tells you the age range!), I would like to focus on some forgotten gems.
I love the Early Ultima Music -- and there are a number of remastered versions of "Stones" that remain haunting. You can compare the original from Ultima V with the chamber verion from Ascenion. It was later featured in Ultima Online. Also people have remastered everything from a vocal rendition to an electronic keyboard edition.
Treasure Quest had an amazing soundtrack that was sung by Jody Gnant. Her website seems down / offline but the album is on iTunes; I particularly love the "Gayatri" and "My little Dancer"...
Music from the 7th Guest was also remarkably effective -- the tango theme that was the seductresses' theme was quite impressive, the haunting theme of the Stauf mansion itself, and the songs that the Fatman and Team Fat did for teh accompanying album are quite amazing. However, check out Skeletons in my Closet, a gem that never made it into the game itself but was on the bonus CD.
Some music doesn't stand as well on its own but is particularly evocative in game context. Take the way that some of these lesser-known spy / hero titles did: The quirky 60s spy-thriller fun stealter shooter No One Lives Forever, which nails the Bond theme style with a smirky joy. A villanous take on the genre is the theme from Evil Genius, also capturing the sense of the right place and time. But the music from freedom Force and its sequel are quite jaunty and wonderfully appropriate: consider the background piece to teh villian Nuclear Winter, for example.
The Heroes of Might and Magic Series have always had some rather haunting pieces. Consider the theme from the Sorceress Castle from HOMM2, this excerpt of campaign music or the Necromancer town from HOMM3. HOMM was based off an older game called King's Bounty, and teh newly done (and highly reccomended King's Bounty: The Legend keeps similar music styles. Listen to this rather beautiful piece from an early continent, particular effective when the violins come in at about 90 seconds, and at 1:50, when teh elven-sounding voice starts singing, it is etherial. Music in a similar vein is the rather unknown gem Majesty, whose sequel, Majesty 2, just came out.
A game few people ever played but whose damn theme sticks in my head -- and you can get the whole game for like 5-10 dollars from Steam -- is Ghost master. It captures teh bouncy Ghostbusters crossed with the Sims mentality down pat.
One last thought for this post: Consider the Disney-esqe moment in the into of King's Quest VII.

I played FFXI for such a long time that the music from that MMO kind of sneaked up on me and I really like it now. Although I no longer play the game I still listen to the music and it brings back some mostly very pleasent memories with a touch of sadness .
