A new difficulty system. Giving you only four choices in difficulty is kind of stupid.
Nightmare too easy? Give the player the option to modify enemy max HP by exact amounts, say 2000%. Is your mage too powerful? Adjust spell resistance for all enemies. Think backstabs are overpowered? Adjust their damage by a fixed percent.
These are incredibly easy to code, and I don't see why they haven't been done. Set up difficulty where you can choose a set of core rules (which would be Easy, Normal, Hard, and Nightmare) and then give you a bunch of variables that you can change. The default for all of them would be one, but if you want to make the game harder (because, really, Nightmare IS too easy, if you have an uber party) you can just say BackstabDamage = BackstabDamage * .85. Or SpellDamage. Or whatever.
