With that said, it kind of complements the lore because ogres come from Qunari broodmothers.

When I saw the new design, something in me sighed with relief. In Origins, if you paid attention during the Brood Mother section, it was explained that beings from all races could be changed into Brood Mothers. The original race of the Brood Mother determined what type of monster it's "brood" of darkspawn would be. Dwarves spawned genlocks, humans spawned hurlocks, qunari spawned ogres. I always wondered why the other races looked basically like demonic versions of their normal counterparts but ogres gained all these freakish hornes out of nowhere. I think this is an improvement to continuity.

so they decided that the best way they could make the qunari cooler... was to completely redesign them. I say that calls for continuity issues. Unless theyve evolved... but blimey that was some quick evolving. I can't see the horns working personally. They could only work if sten had horns and... oh... he doesnt -.-

It has also been stated (by Mary Kirby) that the Qunari breed for excellence, and that they don't descriminate against other races (humans, dwarves, elves) or ability (mages) so long as they follow the Qun (said by Sten in DA, I believe). 

So by that logic, it actually makes quite a bit of sense that some Qunari would be born without horns.  Consider it a genetic fluke based upon bloodlines that are not 100% 'Qunari' due to the mixed breeding with other races.  In Qunari society, you're born to perform a function, not to grow horns =/  so I'm sure a Qunari/elf or Qunari/human mix would be ideal for certian jobs.  Sten could genetically be 'part' Qunari, but consider himself full Qunari because he follows the Qun, and Qunari offspring are, in a sense, raised by the community, not the parents.

Besides that, the horns don't have nerve endings (once again, stated by Mary Kirby) so if they're chipped/broken in a fight, or if the Qunari simply abandon the Qun, it's likely common practive to cut them off and be done with them.  They're not a status symbol.

Sten not possessing horns is hardly an unbelievable loophole for the change in concept design.

proper as in hungarian/transylvanian originating, european folklore based human vampire, of which you can't tell by the looks that it is a vampire

I hate "proper" as in "originally". Stories changes. They're told through word-of-mouth, they're centuries old, and they adapt. Just because we can find the oldest version doesn't make it the most accurate. When I think vampires, I think Dracula. I don't care what the "oldest" version is. Hmph.
