Cameos cost more money. VAs are payed by project, not hours. BioWare would want to get the most out of their money.
Also, their death just denies them from being too important. They can still be squadmates.

People still believe this after all this time? I really need to save the long version of this answer as a template
If they write them as cameos, they still need to bring back all of the expensive voice actors, and still write them into the story in some way. That costs a lot of money for basically no point. Then you have to design a completely new team, bring in voice actors for them, and then develop them over the course of the third game to avoid them being completely flat. ME2 was about developing the characters, ME3 won't be as well. Its as much a money thing as anything else (which is very good for us, considering Bioware seems to be drifting farther towards bottom line hunters.)
This is before even getting into the fan response. Bioware made the whole point of ME2 for the people to find some characters that they liked. They won't just abandon that unless they're a lot stupider than we all think.

there they if you lost you're main character replaced with an unknown person, maybe they use that trick for those who lost team members in ME2.

Thane too. Unless those Ceberus Network news reports are supposed to give us clues about what will happen to him.
But Tali, Garrus and Legion are pretty much impossible to write out. It would make no sense if they left.

Samara and the DLCs are the ones I can see not being back. Samara for the no two aliens of the same race rule. The rest I expect.

Not twelve, that's for sure.
Six or seven would be cool. Keep it simple, and give every character time to really get into the game.

All 9 LIs (of course, excepting Ashley or Kaidan depending on what happened for you on Virmire)

I think Samara's a bit more likely than Mordin, actually. Which could be a shame since I vastly prefer Mordin.
