Honestly, depending on if you are renegade or paragon, there are multiple people that could betray you.
Paragon: Most likely suspects are Jack and Miranda. Jack because you are too goody goody, Miranda because she is trying to protect Oriana.
Renegade: Most likely suspects are Samara and Tali. Especially if you keep the base in ME2. The more you fall in line with Cerberus, the more likely it would be for Tali to pull a "I can't let you do this anymore Shepard" moment. Same with Samara. Eventually, I predict a renegade Shep will cross a line Samara cannot allow to pass.
All told though, my money on an actual betrayal, not a confrontation, is going to be the Virmire survivor. It id very suspicious that the reason the collectors are on Horizon doesn't get captured. I am betting on implantation/indoctrination forcing a betrayal for the survivor.
That would make Jack look very, very, very unintelligent. In that instance I would have to quote my boy Col. Clay from the Loser.

Now Miranda I think would be more emotional and have greater impact because third option would be for to save her sister and get out her out ****ty ass situation... emotional and drama value high. Insights in to more of Miranda character.
I honestly don't want a my previous LI Betrayal storyline thats is soo cliche' and immature I physical get sick because as I have stated for before death negates relationship status. Plus that would really make take a lot of away from Ashley character I just can see her getting Catty **** syndrome. It goes against all the Marine in her. Kaiden well him and Garrus/Thane/ Jacob duke it out and share a beer afterwards. Liara looks like she found herself a Drell name Ferron that took Sheps place.
No for betrayal to be effective it has to come from the place you least expect it, like Council or the Alliance somewhere very high up.
