Too many quarians would eat up too many resources. Which is why they have that one child law.

I don't think that law is regularly enforced.

I had always assumed that the quarian birth restrictions worked in a way that resulted in not much actual policing, the guide line being the only important thing.  They have to keep their population within a few million, not extremely precise.  If one family has two kids, or doesn't have any, that isn't going to affect the entire flotilla.  I assume people live pretty much normal lives until the flotilla drifts dangerously towards one extreme.  If they were right around 20 million people, then the laws would probably be enforced.

I see the likely punishment just being a possible public stigma personally.  Hard to see much more than that.

Maybe they are genetically altered, so they can't have twins.
