Q. about shouts

I'm none too familiar with the "shout" system in NWN2. If a player attacks a placeable that is a member of the "Hostile" faction (such as by bashing at a door or a chest), does that automatically cause a "shout" that alerts nearby Hostile creatures? Or do I need to add some code to the placeable's On Damaged script that will generate a shout?

You will have to put something within the script to get them to respond.

For creatures, default9 (OnSpawn) calls SetListeningPatterns for those shouts. The function RespondToShouts is defined within nw_i0_generic.
