I was just playing ME and died in a very odd way.
I'm on Feros. I saved Lisbeth and was on my way back to Zhu's Hope. I had just beat the second set of Geth ambushers (2 tanks, 2 rocketmen) at the bottom of the rubble ramp between an upper and lower skyway. I had gotten out of the Mako and killed them with my sniper rifle. Ashley had fallen (she decided the best way to deal with the rockets was to try to bash them with her face). Ashley was still down when I got back into the Mako after I had dealt with the ambush.
The Mako was then launched spontaneously upwards and then fell over the edge of the skyway to oblivion.
So, has anyone had anything like this happen to them in ME1 (I haven't played 2 so keep those spoilers out of this thread)? Or any other really weird deaths?

Every now and then, the Mako just does something really weird.
Best I can tell, it's just a glitch and shouldn't happen again unless you're unlucky.

I was killed by a glitch several times. When you're fighting the Thorian, if you go near the edge of the platforms in certain places, you instantly die. I have no idea how a glitch like that even works but it killed me randomly at first and then I was able to recreate it.
I've never had the Mako do that thing in the YouTube video but I did get tangled up with a Geth Colossus once and went spiralling through the air, kind of like what you can do to the Warthog in Halo with grenades and the rocket launcher.
