Apparently you need to do at least 15 missions and 35 assignments to get
the Completionist achievement. I've done way more than that in each
category and still don't have the achievement. Is it given out at the
very end of the game or something?
Also, the Scholar achievement
hasn't activated despite me getting the required entries. I heard on
the Wikia that the achievement is known to be glitched in this way, but
doesn't anyone know what causes it to not activate so I can avoid this
happening on consequent playthroughs?

You should get completionist before the end. I took me more than 35/15 also. I think the game counts them differently. If you did most of the UNC missions, and the whole mainquest, you should get it. I got mine on Ilos only.
Scholar requires you to talk to Captain Anderson at level 1 and ask him about the protheans before nihluus walks in.

Thanks. Well I did get the Prothean entry at the start. I have no idea why it isn't working; hopefully it'll work on my 2nd playthrough. I'd hate to not be able to get 100% of the achievements because of a glitch.
I've now done every single assignment in the game (that you're able to do in a single playthrough) and am about to complete Ilos. Hopefully the Completionist achievement will pop up shortly.

The Completionist achievement tends to unlock around the time you get to Illos. As for Scholar, make sure you talk to every talkable alien during your first visit to the Citadel and ask them about their culture/history, or simply about themselves. Also, after you become a Spectre, you must ask Captain Anderson about his past with Saren, at which point he will mention the batarians, unlocking the respective Codex entry for you (this should be your last Codex entry towards Scholar, after which the achievement will be unlocked). You need to confirm that you asked him about the Protheans pre-Eden Prime, too.

Seems the achievements can just somehow not trigger since I also didn't get Scholar despite getting every entry in the way people here described.

How the hell did Bioware manage to program a game in which you can do exactly what the achievement asks of you but still not get given the achievement? I've never seen this in a game before.

Nope, Xbox. I googled these problems and a few other people have had the same happen to them but no one seems to know why. Thanks for trying to help, anyway. I normally don't care too much about achievements but with the Mass Effect series it felt really nice to 100% them since I'm going to be replaying them so many times anyway. ME2 is the game I have most achievements in by far (only 2 to go, the Insanity difficulty one and Paramour) so it kind of sucks that by default I can't get every achievement in ME1, especially two that are pretty damn easy to get normally.

This is the guide i use. Slightly different to the Wiki one, but it hasnt let me down.
Aliens: Council Races
Asari: Speak with Captain Anderson after the first meeting with the council.
Salarians: Speak with Captain Anderson after the first meeting with the council.
Turians: If you miss it at the beginning of the game, you can get it by speaking to people about Specters and Saran. Otherwise, it should appear after your first encounter with Ashley.
Aliens: Extinct Races
Rachni: Speak with the VI Avina near the Consort Chambers/Krogran Statue about the Rachni wars.
Protheans: Speak with Captain Anderson and Nihlus on the Normandy at the beginning of the game in the Communications Room. This is your only chance in the game to get this codex. If you miss it, you will not be able to recover it later in the game.
Aliens: Non-Council Races
Batarians: After you become a Spectre, speak with Captain Anderson about his Spectre activities at the Docking Bay.
Elcor: Speak with the diplomat Calyn in the embassy. He is located at the end of the hallway to the right of the Embassy Clerk next to Udina's office.
Geth: While on the mission Eden Prime you will receive this codex automatically.
Hanar: Speak with the Hanar Shopkeeper at the Emporium.
Keepers: Speak with the VI Avina in the northwest part of the Presidium. She is very close to the Preaching Hanar and C-Sec Turian whom are arguing.
Krogan: You get this automatically by entering Choras Den. A scene will play showing the Krogran Wrex.
Quarians: Speak with Garrus in the medical center after fighting the thugs.

Thanks for taking the time to help me but as I said, the problem isn't getting the codex entries, it's the game not "realising" I have. I've had them all on both playthroughs. The achievement just won't activate. Same as how I got the achievements for completing 75% of the game using Tali and Ashley but didn't actually get the achievement for completing 75% of the game. They seem to be glitched, it's happened to other people the same way, so I guess I can just say goodbye the the prospect of ever getting all the achievements. ;_;

Maybe clearing your HDD cache will sort it?

I think you need to open them up, not even read them, just press A then B on all of the primary ones
as for the completionist

no, it shouldn't, I have done it a few times with my HDD after installing DLCs

I Play on 360 and have had no acheivement problems at all, so it must be more towards your system rather than the game itself. My son has had no problems either, he also playing on 360 with a different ME disk. I did have a scare with the scholar but as soon as I marked my codex as read it activated. I remember having to complete almost every assignment to get the completionist acheivement. I got the quarian & alliance soldier ally achievements much sooner than the completionist one, which I remember thinking was strange at the time............
