So I'd started a new game of Origins, having completed Leliana's Song and The Darkspawn Chronicles and having successfully gotten the rewards for both Origins and Awakening. And when I start my new game, having completed Leliana's Song, The Darkspawn Chronicles, and now Witch Hunt, none of the DLC items appear. Logging out and then back in didn't work, but when I quit the game to go to the DLC menu, I noticed that while all of the DLC for the tie-in campaigns and items was enabled, I'd forgotten to enable two of my mods--The Ivory Tower and Nightfall Bloom. So I enable them and load the game, and lo and behold, in my inventory: The Ivory Tower set, the Nightfall Bloom set, the Battledress of the Provocateur, Blightblood, the Vestments of the Seer, Sorrows of Arlathan, and the Dragonbone Cleaver. I don't really know what caused it to start working--logging out and back in, then reloading the game or enabling the mods--but I figured I'd post it just in case it helps someone.

And then I ran into a bug--one I'd experienced before, so not caused by this--where stealthed enemies don't reappear when attacking you. Since I lacked a mage--thus no area damage spells to force them to reappear--I used the console to turn Ser Jory into a mage. Apparently this wipes your inventory, so I quit and had to reload from an earlier save, and there go my DLC items.
I can now confirm that the Ivory Tower etc. has nothing to do with it. For me at least, loading a save, logging out, then logging back in, then reloading the save works perfectly. The problem being that it somehow flags your DLC as unauthorized, so you have to exit the game and restart it. Annoying--props to Bioware for finding this many ways to make me hate their playtesting team--but at least it works.

That sounds a lot like the trouble I was having. One of the hallmarks of which was that the DLC achievements on which the equipment depends were glitchy: they'd show up highlighted, but when I actually clicked on them, would be listed as "Locked" until I logged out and in.
Turned out my CD key hadn't been installed properly. this post (in the thread which I linked to earlier in this thread) has further explanation & links.

The only sure way to resolve this issue is to have a save from Leliana's Song in the Chantry, just before Leliana gets the last piece of the Battledress. Make sure you're logged in and acquire the last piece - this will reset the achievement and any new characters will get the item - any games without the item will have it once they are loaded. This works for me 100%. However, simply using the cheater console command ("runscript cheater") does not work, even if logged in. You'll have to use this method.
This is the only DLC item that is affected in this way - all other DLC items are properly authorized. Maybe in the upcoming (and presumably final) patch this will be looked at, but I think it's far too late to make a difference.
