Without spoiling anything for me, can someone tell me if I missed the boat on getting a dog?
I know when you get to Ostagar you get a quest which I'm assuming will get you a dog, but I never finished it - I moved on with the main plot and now the place is destroyed. Can I ever get a dog or am I screwed?
Did I miss out on my dog?

pretty sure you are screwed. If by the time you get to lotherin you haven't got him, he is gone. Probably in some dark spawn cookpot. You are a evil evil man.

Yeah, sadly you have to do the quest to have the dog which is bit of a bummer if your trying to get some items/treasures only he can get.

Bah! I don't know if I'd even use him much, but I wish he was at least chillin' in my camp. Ah well...

I didn't use him very often I'd just switch him out when i wanted him to look for items. He does have a very useful stun ability though if you do feel like using him in combo with some aoe mages.

Do-Over. I would if it was only 4 or 5 levels back. I actually went back a few saves and gave him his cure after I found his pen deserted after the attack. Dog is your best friend and as good as any ally. And you'll miss some great one-liners.

Is there a dog other than the one you get with the Human Noble origin? I had a quest in the first 15 minutes of the game to get him.

I thought I read you can get one last chance via a random encounter where you find a dog fighting darkspawn?

Any origin story outside of Human Noble gets their dog in Ostagar I think. Remember when you delivered that flower to the kennelmaster to help him cure his dogs?
If you're not a human noble, he asks you to muzzle the dog. Apparantly he then imprints on you. When you then leave Ostagar (after Flemeth has resqued you) he will appear with some darkspawn, kill those and you have a new Mabari.
