IMPORTANT: Unequip all weapons from your characters before looting back your gear.
At a certain point you lose all your equipment. You regain it soon after, however I missed the chest containing the items. So instead I ran off unequipped and punched my way to some mediocre gear. I equipped said gear which included weaponry. I ex-filtrated the area and had almost all of my equipment returned to me despite missing the chest containing it.
I got everything EXCEPT my main characters weaponry, the only way I got round this was to reload an earlier save and remove all weaponry before retrieving my gear. Had I not had a prior save my weapons would have been lost to the fade, so to speak.
I have also noticed a similar problem with the Red Dragon Armour (DLC), sometimes the item has vanished from inventory completely. For example in the Dwarf Commoner origin story you are arrested and have your gear taken from you, it is never returned and thus the chest piece disappears.
Felt I should let people know. On a lighter note Dragon Age is a veritable work of art in a market populated by many "average" RPGs!
