To answer the thing about Zevran trying to kill the warden twice (very late) -- this does not happen, though the toolset implies that originally the Zevran ambush was the second encounter with him. There are actually two references in the game to the second attempt -- once from Alistair, and one from Zevran (where if a female warden rebuffs him, he answers something like, "Try to kill a woman twice, and she gets all icy."
This actually makes sense. Not just because of the dialogue that got left in, but because Zevran's assassination attempt always
felt like a second encounter to me. Call me crazy, but just the way Zevran took the trouble to get a random third party to lure the Warden into a trap and then steps foreward as if his handsome mug is some big reveal, smirks at the Warden as if they're old adversaries, comes crazy prepared in that way one does only when they failed the first time, and cries out "The Grey Warden dies here!" as if the Warden failed to die somewhere else (well, they did, but not by his hand, which the subtext seems to imply) just screams "second encounter" to me.
Not to mention the subtext between Zevran, Warden and the other companions after he wakes up. The way Zevran and the Warden talk to each other contains much more fluidity and familiarity than they would have if they just met face to face for the first time. Not to mention Alistair calls Zevran "
the assassin" instead of "an assassin," which implies... well, you get the idea.
