Nope, Cousland and Alistair do not repopulate Ferelden all by themselves. One of the more nauseating fanfic aspects out there is the lady warden having tons of kids with Alistair or any other random NPCs in the game. Maybe with a partner who is not tainted, like Zevran or Ser Perth or Teagan, it's possible, but not with Alistair. I get the feeling those fics are written by women who took offense at Alistair for dumping their 'tainted womb' warden and they just had to prove, if even in their own lore contradicting head canon that Alistair was wrong and they could have multiple children.
All circumstances of leadership lead to another succession crisis.
Anora married to Alistair: No child because one is potentially barren, the other tainted, no heir
Anora married to Male Cousland: Same situation as with Alistair
Alistair married to Female Cousland: Both are tainted, no heir
Anora solo: Never marries because of Daddy Issues, no heir
The older a healthy woman gets, the more markedly her fertility drops and we don't know what damage that is quite possibly irreversible is done to her lady parts by being a grey warden. Lady warden is pushing 30 or more years old by the time the events of Inquisition happen. And we don't know if there even is a cure. After all, no one has tried and succeeded in 1000 years. Of course, the PC snowflake might just pull it off because: Player Character Specialness. But I'm not going to hold my breath. Bioware loves to monkey with the story. I get the feeling the 'cure' was just a fan appeasement anyway, with no real consequence. Or DA4 may just prove me wrong and it causes a stir in the ranks of the Wardens. Only time will tell.
The more Bioware messes with the warden the more it fubars up the retirement scenario I had in my head of my Surana, childless but happy and fulfilled helping King Alistair run Ferelden.
