It worked for me. I had to get her approval up and then on my way back from the blackmarsh it triggered it.

I know for sure now that you need to have soldiers sent to protect trade routes for Velanna's Exile to come up.

I've done that on many of my playthroughs but was never able to get the quest until I went to protect trade.

No. That is a myth.
Even the Wiki says that protecting the trade routes does *not* aid in triggering the quest.
Apparently, any choice there can trigger it... or not, as the case may be.

Velanna's Exile still won't start for me. Doesn't matter where we are, where we go, her approval, pre/post-Joining, ANYTHING! Patch 1.04 did not fix it. If anything, it screwed up the Peasant Revolution quest for me >_>
As for Sigrun, I've never had a problem with her quest.

well see, I have only playthrough under my belt but I couldn't get Sigrun's question to trigger. But I imagine it could be that I picked her up last and the game pretty much forces you into to start the beginning after you complete the last of the main plot storylines.
