I played through Mass Effect 2 a second time to get the No One Left Behind achievement. I had everyone's loyalty and I chose the right tasks for the people but I still did not get the achievement because Tali died. I don't know how that could happen but I have a feeling it had to do with us getting romantically involved.
Could someone please help me out before my next try?

It didn't show how she died. In the final cutscene as you're running back to the Normandy I think the camera just passed over her body.FieryPhoenix7 wrote...

The hold the line sequence has a point value system, in which Tali, Jack, Kasumi, and a few others have an extremely low score of 1. If you leave any of them to hold the line they will most likely die. Best bet is to go have a look at the guide stickied, it explains it.

Don't bring grunt zaaed jacob or garrus since their the best at holding the line or just bring tali with you to fight the last boss

Taking any of the heavy hitters like Grunt, Garrus, or Zaeed to the the final boss fight OR sending them back to the Normany with the crew endangers the weaker members (Mordin, Tali, Kasumi, Jack) in the hold-the line section.
Send someone weak (Mordin) back with the crew. Don't take the beefier ones with you to fight the human reaper. Everyone should get out safely.

Yes; you don't actually have to leave all three behind. You can alway take one of them to the final boss, as long as the other two are loyal and are left holding the line. So, at least two of them must be left behind, but not all three.Gibb_Garrus wrote...

I'd keep that save handy...I purposley killed some people in my playthrough for story tone (no one dying on a suicide mission is dumb) killed jacob as tech expert, zaeed as fire team, and tali died holding the line
