The game tells us that reapers are hybrids. They are both organic and synthetic. They use the traits of other species and the best technology present to improve themselves. Therefor assuming that they were created is not very logical. It's more likely that they were once organics who used technology to enhance themselves. Sovereign/Nazara could also be speaking the truth when he said their species had no beginning and no end. It reminds me of the so called Omega Point theory (pioneer by Teilhard de Chardin and later expanded by John David Garcia, Frank J. Tipler, and Ray Kurzweil). It's a theme also explored in science fiction.
In short the idea is to become all knowing and immortal. Much like a god or God. To reach that state we could use technology. That state is the ultimate goal and is called the Omega Point. It is uncertain if we can reach it. So the literature is talking about "approaching" it instead. It's complicated stuff. Try the link above if you are interested.

Kaiden/Ashely were there when the swarm was coming and it never shows them getting away. He/She was out there in the open with all of the other villagers. The only person there who escaped was inside a sealed bunker. And if the the illusive man is right, they targed that colony because one of your former teamates is there. Rather suspicious that they were one of the few who weren't taken away.
The Collectors just have to implant some sort of controlling technology that's too sophisicated for Alliance to find, and then a bit of amenisa to the victom and bam... They've got a sleeper agent who has close ties to Shepard and he/she doesn't know they're a sleeper agent.
