Hello,
So, I really wanted to avoid having to post this as a "technical problem", but given extensive testing and observation, I am almost certain that there is indeed an actual bug with the loyalty system, specifically in reference to "loyalty conflicts" and the paragon/renegade requirements to overcome them.
My biggest concern is the Jack/Miranda conflict, which seems impossible to overcome no matter which variables I change. I have read online that people have been able to choose the "mediator" option and not lose either of their loyalties during the argument, but the Paragon they claim to have is lower than amounts I have had that turned out to be insufficient. I tried siding with Miranda, and it seems that even having Paragon in the 90%-ish levels will not win back her loyalty. I have tried a few different ways, and it seems impossible to leave for the Suicide Mission with both of their loyalties. Forums already existing have been useless, as the few people who got it right all give different opinions on why it worked, while those for whom it has not worked all have different reasons why they think it didn't. I am posting this because I want actual support staff to account for these problems. Mass Effect 2 is an amazing game, which is why a problem like not being able to finish the game with a happy ending is so endlessly frustrating.
BioWare, if this is a bug, PLEASE fix it. If it isn't, then please give an official notice about what we are all missing, because I assure you that many people are experiencing this exact same frustration, and trying to over-analyze the game mechanics themselves, to this extent, can start to ruin the game.
