In the Chantry, after I tried to open the drawer with the last armor piece, everything went dark and after a few twisting, I realized Leliana zapped behind the walls of the building. She was able to go all around the builiding and a a few openings to see inside the Chantry... but nothing big enough to squeeze out of there. I loaded the autosave file to start over and chose a diffferent angle to open the drawer with success that second time. Quite a trap!
