Do all MMOs have subscription fees? Or even subscriptions?
No they don't all have subscriptions. One of the better "just buy the box" games is the Guild Wars franchise. I only know from GW1, but there is a PVP focus, hence the name, but you can certainly just focus on the PVE aspect, and even play most of the game solo if you want as there are helpful party AI companions to tote around.
Many smaller MMOs have various methods of earning money: there is the popular micro-transaction method where the basic game is free, but you pay for character upgrades, quest packages, and so forth; there are also ones where there are a few tiers of gameplay where you have a basic game, and you purchase the higher level content as you come to it; and there are also the real economy clone games where you use real-world currency to exchange for game currency, the company takes a cut, and people can profit both inside and outside of the game world.
However, I will add that when playing an MMO by a massive company like Blizzard, quality really shows. There are several dozen servers (many of which got recent hardware upgrades), a huge forum community where posts move so fast it's hard to keep up and things fall off the front page quickly, a couple of official Blizzard partner fan resource sites with all of the free information and addons you can possibly imagine, and more.
My first experience with a huge online fan community for a game was Bioware's own Neverwinter Nights hub, as well as the fan module sites. At the time, I thought those were enormous. WoW's far eclipses those.
The main problem an older MMO like WoW has now though is that there are now three expansions, in addition to the original game. It's a heavy price for someone who is new to MMOs or even the Warcraft universe. They occasionally have store deals and such, but it's still quite a bit in addition to a monthly fee. I found the same true of Guild Wars in a way: I had gone away from GW for a while, and in the meantime they had released 2 additional campaigns and an expansion. When I was browsing forums and such for builds, people list all sorts of skills that I didn't have access to since I didn't have those other 3 games.
There's something for everyone really. But it all comes down to how much you are willing to pay for entertainment. If you want to spend hundreds of dollars on a single game over years, or hundreds of dollars on several games over years.
