Can't find the 4th vale tear. DId the summon circle. got the one on the right side, top by the summoning circle, just above the blocked main gate. Couldn't find any others so went through, killed the baroness, went to finish off the left side of the "real world" and got caught on the vale tear there.

If it has anything to do with the dragon bones, I filled all the slotsHow do i get to that one in the fade?

Any help would be greatly appreciated.

I’m trying to mentally visualize the layout and if I’m reading your question right, there’s one more to the left of the summoning circle, its the same one that I often forget, theres a big tree blocking the view from the main path.
