Is there a helmet that matches the Warden Commander Armor?

Unfortunately no. As you're likely aware, nothing unique to that DLC will import into Awakening. If you have the PC version, you can use mods to import WC Armor into Awakening...however that helmet I mentioned won't look right in Awakening even with mods because of a particularly annoying bug I detailed here: http://social.biowar...41453/1#4570107

The sword is Starfang (from Warden's Keep DLC). The shield is Bulwark of the True King...it's one of those promotional items, think you got it by registering with Alienware or something like that.

Actually, after patch 1.04 all Ostagar, Honnleath and Warden's Keep stuff imports to Awakening. And my helm looks fine.
Michael Dryden has a "Warden Commander Helm" which could be the match for the Warden Commander armor. Interestingly, it is identical to the Helm of Honnleath except for a slight variation in color. For me, Honnleath is the "official" complement to the WC armor set.

I have Patch 1.04 installed on the PC version, and I can confirm that Warden's Keep stuff does *not* import into Awakening. I've not heard of this being changed, and it certainly wasn't mentioned in the patch notes.

In other words, if you import with starfang, you'll lose it but you'll get Warden's Reach

Me too. One of the best helms in the game. I usually don't like the whole face hidden but make an exception for this one.
