This is my experience with DA:O romance threads.
 
My char is city-elf-male. I have managed to seduce both Morrigan
and Leliana paralel and kept sexually active state with both of them. I
am sure this is not a bug, but an intention of the developers! This is
my story:

 
Morrigan:
 
I started with Morrigan. I increased the approval rating to almost
max with gifts and talking. I missed the first opportunities to start
romance (It is possible in an early game converstion and in another,
later conversation as I found it later here: http://dragonage.wik...igan's_Dialogue ).

I statred the romance Morrigan with Golden Mirror gift (can be found in dwarven city merchant: http://dragonage.wik...i/Golden_Mirror ). Important answer: "It is simply a gift for a beautiful women...".

As my approval was already high with her at this time, after giving
her the mirror, the conversation started which initiated the sexual
interaction and cutscene ("It is so cold in my tent, all alone...")

 
Comments: Do not make Morrigan personal quest (killing Flemeth;
actually you can get Flemeth's grimoire without killing Flemeth and
possibly increase Morrigans approval with it)! After killing Flemeth -
if you are in romance with Morrigan - she fell in love to you and will
not go to bed with you any more, becaused of her confused feelings!

You can not (!) start/restart romance with completing the killing Flemeth quest!
You can restart romance with Morrigan with Golden Mirror gift!
 
Leliana:
 
Paralel with the above and after that I continously increased
approval rating of Leliana also with talking and gifts. Telling the
truth, I don't know when I started romance with her (I think with a
conversation option. You may find hints here: http://dragonage.wik...iana's_Dialogue).

With romance active and high approval a jelaousy conversation was
started with her ("...you are still with Morrigan..."). Here I broke up
with Leliana and stayed with Morrigan (romance stopped with Leliana).

It is important, that this was before I started Leliana's personal quest (Leliana's past / Marjoaline).
After the brake-up I completed Leliana's personal quest. After the
personal quest in the following quest related conversations I hardened
her (hardening is important - see later!; hints for hardening here: http://dragonage.wik...iana's_Dialogue) and restarted the romance with her (also based on these convesation)!

This restart option is hard to find in the conversation, becouse
there so many conversation options and there is only one option which
restarts the romance!  The important conversation starts with: "Are you feeling better about what happened with Marjolaine?". More hints here: http://dragonage.wik...iana's_Dialogue. The important answers:

• I can only hope to one day be as special as she was to you. (Confesses her love for you. This starts romance)
• That will not change, not if I can help it. (Confesses her love for you. This starts romance)

example conversation:
- "We need to talk."
- "Are you feeling better?"
- "Er, I think I see what you mean."
- "She was special to you wasn't she?"
- "I'm sorry it ended to badly."

- "Everyone changes unfortunately."
- "I can only hope to one day be as special as she was to you."
- Romance initiated
 
After restarting the romance I increased the lost approval
rating and with enough approval (love) a conversation was started with
her which initiated the sexual interaction and cutscene.
 
Comments: Jealousy dialog do not come up twice! This is why -
with stopping the romance at the first jealousy dialog and
restart romance later - the second time you can reach the sexual
conversation without jealousy.

 
Morrigan after having sex with Leliana:
Going to Morrigan after having sex with Lelliana (and
possibly even in romance state with Lelliana), initiates a jealousy
dialog with Morrigan. Here you should tell Morrigan, that you stay with
her and brake up with Leliana! This way you keep the sexual state with
Morrigan. Morrigan will not come up with this again! (Jealousy dialgos
do not come up twice!) And also, Lelliana will not reflect to this
anything, just keep the sexual state (becaouse Leliana's jealusy dialog
was also initiated earlier).

 
General comments:
I am sure this also works the other way around
(stopping/restarting romance with Morrigan and keeping romance with
Leliana). Just keep in mind the the last chance to restart romance with
Morrigan is the Golden Mirror and the last chance to restart romance
with Leliana is the conversation after her personal quest! It is also
enogh to stop romance with one of the partners, it is not necessary to
brake up with both! This is not necesarry to initiate both jelausy
dialogs before restarting romace. It is enough to initiate jealousy with
one of them and leave the second jealousy to the last step. I think
this theory can be applied to any possible love triangles.

 
Having foursome with Leliana, Zevran and Isabella (at Denerim / The Pearl):
After Leliana is hardened (see above!) take her and Zevran to
The Perl at Denerim and talk to Isabella. Suggest Isabella to get know
each other at her ship Hardened (!) Leliana join you to keep and eye
on both of you. Isabella/Zevran ask you if Zevran can also join!
THERE IS NO CUTSCENE FOR THIS, but it still fun!. You can do this
conversation only once!

 
Comments: Threesome is also possible with Alistaire and female warden.
 
Other sexual encounters: You can seduce Gheyna (elf girl) in Dalish camp. Hints here: http://dragonage.wikia.com/wiki/Gheyna
I unfortunatelly missed this opportunity, as after finishing
the main quest here, Gheyna's boyfriend, Cammen leaves the camp. The
quest in wihch you can seduce Gheyna can be started with Cammen! So make
the Cammen quest before finishing the main quest here! There is also no
cutscene for this!

 
Comments: Cammen can be also seduced by female warden.
 
Possibly I could have keep sexual connection with Zevran also, but this is not my style
 
An intereting challange can be the following: being a female
warden keeping active sexual state paralel with Leliana (bisex),
Alastair (hetero) and Zevran (bisex)! ))
Anyone tried that?
And what if you take this three to Isabella? (according to
the info available, there is no fivesome, but anyone tried it? )))

You don't need to be in a relationship with Zevran for him to participate with...er..activities with Isabella. He'll only refuse if the other two are Wardens, i.e., female Warden romancing a hardened Alistair.

However, as far as I'm concerned with my Elissa Cousland taking all three along (in love with a hardened Leliana and in love with a hardened Alistair; and for some reason both relationships were active through to the end of the game), the Leliana relationship overrode Alistair in the Pearl. (Though the transfer to Awakenings had the Alistair relationship override Leliana's. Go figure.) Maybe it's the order I chose my party, but that's what happened in mine.

Oh, and if you're a female Warden in a loving relationship with Leliana, take Oghren along to the Pearl with you. Hilarity.

Have sex with morrigan after Lothering, and wait to get the mirror after you break up. That's how I got both Lel and Morrigan.

Yes.  Well, I know you can if you seduce Cammen.  As long as you don't mention it!
