I had this problem twice when rescuing my companions. After rescuing Alistair in the Fade, I talked to him again, and he disappeared. Then when I moved it looped back to before he disappeared and I had to go through the conversation again. Later, I rescued Wynne, fought the demons, and talked to her. After I finished that conversation, the game looped back to the conversation you have before the fight (I didn't have to redo the fight, just the conversation). It looped through that conversation two or three times when I tried to talk to her. When I tried leaving, the correct conversation played.

No. No toolset and no patch--all I have is the D/L content that came with my preorder.

same thing happened to me, i think it's a scripting prob. sometimes if you wait before talking, the event will fire like it was designed.

Same thing here, also in the Fade but after rescuing Morrigan, I talked to her like 3 times AFTER killing her "mother" and she kept repeating the initial dialogue (with mom included), I walked away and then the correct cutscene showed up ... go figure ...
