I'm playing before playing Awakening, so it's pretty hard and the dwarf already bit the dust once. Does any character dying affect your ability to get achievements? Are there any decisions you can make or things you can miss that affect the achievements? I mean, after all, you get your rewards for this DLC based on them...

Nah you can die as many times as you want during this DLC. You get the two first items simply by playing through and finding all codex entries (just click everything).
The last and most valuable item you get for beating the final boss on Hard or Nightmare. Nothing about deaths whatsoever, just have to beat him.
Note that you don't even need to play the whole thing on Hard or Nightmare. Just the final boss.
