Note: I've only played the Xbox 360 version, I do apologize if any of these problems are also persistant in the PC / PS3 versions.
After recently replaying through the game these past two days in order to brush off every codex entry, sidequest and important choices (explained at the bottom of the post, not really important for the suggestions) I've found a lot of tedious minor things that would have made my life a lot easier if they were fixed. I'll just add them here. Please don't harm me if these don't seem minor from a programmer's point of view, I'm saying this as an armchair developer with no proper expert game programming knowledge.
Console version suggestions:
NEW: Deleting characters.
If you try to create a character after you reach the limit, you cannot delete them in the game itself and you require to go into the memory files and delete it from there. This is especially troubling when all your characters are the same name and you're extremely paranoid of deleting the characters you've created. An ability to delete the characters from the game itself (similar to the Mass Effect 2 mechanic) would probably be one of the best.
"Item recieved".
My problem with this text isn't from when I defeated enemies and looted them or something, the problem for me is when a quest giver tells me "Take this, it's valuable!" and then it tells me I recieved an item. Upon opening a menu, I have no idea what I've recieved and cannot say if it's valuable or not. This is even more of a problem when you're given important items that might have quest purposes. A way to know what item you've just recieved or how to easily track it down in your bags and spot it would be amazing and extremely convient.
Edit: Oh, I've found the perfect example. You pickpocket somebody, "item recieved" and you have no idea what you just stole.
Scrolling through Codex entries.
I've just found a note on the floor and I have looted it, I press the back button to open up the codex entry and read what it might say but for some reason it decided to open up my inventory. I think to myself that I'll just simply read it from the codex. There's a dozen or so new codex entries in the "Note" tab so I open it up and the tab goes further than my screen shows. In this scenario, I found myself wanting to have the right analogue stick raise / lower the codex entries so I can see where exactly my new note codex is located. Browsing through the entire Quest-Related tab, Note tab and such when you're not even certain the codex entry is located in that specific tab is troublesome.
Approval bar.
While I'm basing this off what I've heard from PC users, I have heard that you can scroll over the approval bar and it'll tell you how the other person feels towards you like if they are a "friend" or something. While on the Xbox 360 we can ballpark how the person feels for us with the bar itself, being able to see the approval bar itself (if what I heard is true) would be extremely nice. Far too often did I have to sit there and wonder if the approval bar went over to the next "level" or not.
Suggestions in general:
Recent Codex entries.
Following the above, often would I find the need to scroll through dozens of already read Codex entries to finally find the one I had been searching for. When I opened my codex to only read through 1-2 newer entries, while going through the 200-300 others (again, the above scrolling would've helped tons), I'd find myself wishing there was another tab that was completely unlinked from the others. A tab that would show all recent unread codex entries since the last time you had opened the codexitself. This would allow me to go through entire scenes (like the defense of Redcliffe for example) without stopping and letting the mood calm down while I read the codex. I can play through the entire area, open the recent entries and find what happened.
Adding more opportunities for changing party members out of camp.
I know there isn't a camp system in Dragon Age (instead using Kirkwall as it's base of operations from what I heard) but needing to go back to camp and then back to the location you came to swap out a party member was very inconvient. When doing the Wynne's Regret quest, I had forgotten Wynne in the camp and had to go through the loading screen to the camp and then the loading screen back again to the Dalish Camp to simply replace Dog with Wynne. While I do understand why this is so, adding more opportunities to change party members would be extremely helpful. The many times I wish I could swap a character in Orzammar and Denerim (areas with barely / no fighting) were many.
Using party member skills when not in use.
In Awakening, I had decided to make Sigrun my Runecrafting partner. This worked well until I realised I needed to buy things in Vigil's Keep Throne Room and then go out and make her craft it for me over and over again until I recieved everything I desired. This made the crafting really tedious and time consuming for going in and out of the keep every two minutes in order to purchase materials and using them. The ability to use their skills when in the throne room itself would've made it a lot more convient and less dreadful.

(the two days replaying the game thing)
I had went into a self imposed exile for two days in an effort to complete Origins and Awakening 100%. Except for the few bugs preventing me from doing a handful of sidequests, I've completed it and read every codex entry. These above suggestions were the small things that made me sigh as I went through the game.
I've got a save at every single important choice in the game for the sequel in an effort to not miss anything.
