I have defeated Harvester on Hard, but did not get the achivement. I changed the difficulty to fight before entering the room with the Harvester, and I have not changed it at any point during the fight.
Anyone can say why can it happen? I really don't want to try again if it will be another wasted effort. :(

You have to play the whole game either on hard or nightmare to get this. If you change the setting you will not get it.  Beating the Harvester is perhaps the hardest foe in all of Dragonage so you better came fully prepared and equipped.  Spoilers prevent me from saying more on this subject.

Thanks for your reply, but that's not true. You only have to beat the Harvester itself on Hard, rest of DLC does not matter. It has been confirmed numerous times by people on this forum plus on Dragon Age wiki. Don't worry about spoilers, I beat the DLC couple of times now.

By the way, I have the same bug with the Witch hunt monster.

I am basing this on my experience with the game so I might be incorrect, but the one time I played the game  where I changed the settings from normal to hard to fight the Harvest I did not get the specific achievement.  I agree with you about the other ones, but I was not writing about them.  I had no problem with WH.

I played WH on Hard and then on Nightmare both on PC and XBox. I found it fairly easy, much, much easier than GOA. The final monster at the Dragonbones was the most difficult fight in it. It reminded me of the Phantom Dragon in Awakenings but with some differences. I found range weapons very effective againt the monster as it was more focused on the others attacking it and was ignoring the archer. I even had my Arcane Warrior-Blood mage whom I imported from GOA shooting arrows at it with the Bow of Arlathan you pick up along the way although both the Dog and Finn went down quickly. By the way, you can get runes in the armor and weapons of Ariane if you sell them to Sandal and leave them while you do something else and then buy them back later. Another thing I discovered and was surprised by (and I found this in GOA), is that you can use the Robe of the Architect as a rune stone, at least when playing as a Arcane Warrior. I was actually able to place ROA in two different sets of armor, the Sentinel and the Golem shell. (I should add that these robes were already in the imported Warden but as runes of amplification.  It was not until I moved them from one armor set to another that I found out they were the ROA as it turned out I actually had two ROA's.) It would have been even better in the Battlemage armor, however, but I found that you can not make this change in WH as the Robe will vanish. Usually, I don't kill the Architect when I play Awakenings but my Blood Mage/Arcane Warrior did. Since I knew I was going to play GOA next, I went back to the Vigil and made a save to use for GOA, and then returned to kill the Mother. After the game I loaded this save and made any changes and restocked my character to use in GOA and saved again to use this save in GOA. I also made a save just before fighting the Harvester to use in Witch Hunt.

I don't know if you're still working on this achievement or not ... but I've found from my own experience that you can do the entire add on pak like on casual for example... but right before you go into that back room where the Harvester crawls onto the big hulking body, you can change it to hard... but after that cinematic you cannot change the difficulty or you cancel any achievement other than just 'Reaper'...
